MENSURAÇÃO DAS PRESSÕES INTRA-CUFF DE VIAS AÉREAS ARTIFICIAIS DE PACIENTES INTERNADOS EM UMA UTI GERAL ADULTA

RESUMO

A Ventilação Mecânica (VM) é utilizada como suporte durante a terapêutica de pacientes com insuficiência respiratória com o objetivo de manter as trocas gasosas adequadas. O tubo endotraqueal possui em sua extremidade um balonete, ou cuff. A pressão do cuff deve ser monitorada periodicamente, a fim de proteger o paciente de importantes complicações. Assim, o objetivo deste estudo é verificar a prevalência de alterações das pressões intra-cuff das vias aéreas artificiais de pacientes internados em uma UTI geral adulta de um hospital universitário no sul do Rio Grande do Sul. Trata-se de um estudo prospectivo, descritivo, longitudinal com abordagem quantitativa. Os dados foram coletados no período de agosto quando necessário, correção das pressões intra-cuff foi utilizado um sistema composto por um manômetro analógico, acoplado a uma torneira de 3 vias descartável e a uma seringa de 5 mL. Os resultados mostraram um total de 493 aferições. As pressões intra-cuff encontradas variaram de 0 a 184 mmHg, com média de 21,6 ± 23,4 mmHg. Do total de medidas obtidas, 48,5% (n=239) dos balonetes estavam hipoinsuflados (menores que 15 mmHg), 31,6% (n=156) adequadamente insuflados (entre 15 e 25mmHg) e 19,9% (n=98) hiperinsuflados (maiores que 25 mmHg). Conclui-se que mesmo com o controle sistemático, a ocorrência de alterações nas pressões intra-cuff são frequentes. Assim sendo, uma rotina de mensurações diárias deve ser implantada nas unidades, bem como o treinamento da equipe assistencial, protegendo assim o paciente de importantes complicações, como a pneumonia e as lesões traqueias, por exemplo.

ABSTRACT

The Mechanical Ventilation (MV) is the basis for supporting the therapy of patients with respiratory insufficiency in order to maintain adequate gas exchange. The endotracheal tube has at its end a cuff. Your pressure should be periodically monitored in order to protect the patient from major complications. Therefore, the objective of the study WAS verify the prevalence of changes in the intra-cuff pressures of artificial airways of patients hospitalized in an ICU of a university hospital in the south of Rio
Grande do Sul. It is a prospective, descriptive, longitudinal study with a quantitative approach. The data were collected in the period from August to October 2016, using standardized instrument. For the measurements,and for corrections of intra-cuff pressures when necessary, a system composed of an analog manometer coupled to a disposable 3-way tap and a 5-mL syringe was used. The results showed a total of 493 measurements. The intra-cuff pressures ranged from 0 to 184 mmHg, with an average
of 21.6 ± 23.4 mmHg. Of the total number of measurements, 48.5% (n = 239) of the cuff were hypo-inflated (less than 15 mmHg), 31.6% (n = 156) adequately inflated (between 15 and 25 mmHg) and 19.9% (n = 98) hyper-inflated (greater than 25 mmHg). It can be concluded that even with the systematic control, the occurrence of changes in intra-cuff pressures are recurrent, and therefore a routine of daily measurements should be implemented in the ICUs. Also, the care team should be properly trained in
order to protect the patients from health complications such as pneumonia and tracheal injuries.
